Effects of Covid-19 Pandemic on General Surgery Emergency Protocol

INTRODUCTION: Together with the Covid-19 spread in Turkey, the hospitals used for the treatment of this disease has been risk zones. In this context, restrictions have been made in health services, especially in surgical services. It has mostly led to changes in treatment protocols in emergency surgical interventions, as well as postponement of elective surgery cases.
In the study, we aimed to reveal the effects of the COVID-19 pandemic on the general surgery emergency protocol after the evaluation of emergency general surgery cases between March, April, May 2019 and March-April-May 2020.
METHODS: The dependent variable of the study: March, April, May, the period of before Covid 19 (2019 year) and the process of after Covid 19 (year 2020).
Independent variables of the study: age, gender, comorbidity, operation history, hospitalization diagnosis, treatment method (medical, surgery), length of stay in hospital and complication status.

RESULTS: In paired comparisons in the study, age (p = 0.338), gender (p = 0.841), comorbid disease (p = 0.076) and operation history (p = 0.354) were found to be statistically insignificant; The type of treatment (p = 0.033), length of stay in hospital (p = 0.015) and complication status (p = 0.046) were found to be statistically significant.
DISCUSSION AND CONCLUSION: As a result, general surgery applications differ during the Covid-19 period. During the pandemic process, the prominence of medical treatment, shortening of the hospitalization period and the increase in the frequency of complications are the main results when compared to the previous year in general surgery emergency cases.

Covid-19 Pandemisinin Genel Cerrahi Acil Protokolü Üzerine Etkileri

GİRİŞ ve AMAÇ: Türkiye’de COVID-19’un yayılmasıyla, bu hastalığın tedavisi için kullanılan hastaneler bulaş açısından riskli bölgeler ilan edilmiştir. Bu bağlamda verilen sağlık hizmetlerinde de, özellikle cerrahi hizmetlerde kısıtlamalara gidilmiştir. Çoğunlukla elektif cerrahi vakalarının ertelenmesinin yanı sıra, acil cerrahi müdahalelerde tedavi protokollerinde değişime gidilmesine yol açmıştır.
Araştırmada, 2019 Mart, Nisan, Mayıs ayları ve 2020 Mart-Nisan-Mayıs ayları arasında acil genel cerrahi vakalarının değerlendirilmesi sonrasında, COVID-19 pandemisinin genel cerrahi acil protokolü üzerine etkilerini ortaya koymayı amaçladık.
YÖNTEM ve GEREÇLER: Araştırmanın bağımlı değişkeni: Mart, Nisan, Mayıs ayları Covid 19 öncesi (2019 yılı) ve Covid 19 sonrası (2020 yılı) dönem.
Araştırmanın bağımsız değişkenler: yaş, cinsiyet, ek hastalık, operasyon öyküsü, yatış tanısı, tedavi şekli (medikal, cerrahi), yatış süresi ve komplikasyon durumu.

BULGULAR: Araştırmada ikili karşılaştırılmalarda yaş (p=0,338), cinsiyet (p=0,841), ek hastalık (p=0,076) ve operasyon öyküsü (p=0,354) istatistiksel olarak anlamsız olarak bulunmuş iken; tedavi şekli (p=0,033), yatış süresi (p=0,015) ve komplikasyon durumu (p=0,046) istatistiksel olarak anlamlı bulunmuştur.
TARTIŞMA ve SONUÇ: Sonuç olarak, Covid-19 döneminde genel cerrahi uygulamaları farklılık göstermektedir. Pandemi sürecinde genel cerrahi acil vakalarının önceki yıla göre kıyaslandığında medikal tedavinin ön plana çıkması, yatış süresinin kısalması ve komplikasyon sıklığının artması öne çıkan temel sonuçlardır.
