Compare the effectıveness of usıng Hydrofiber dressing with silver and conventional treatment on wound healing in pilonidal sinus patients operated by lay-open technique

INTRODUCTION: This study was aimed to compare the effectıveness of Hydrofiber dressing with silver and conventional methods on wound healing in pilonidal sinus patients operated lay-open technique
METHODS: Sixty patients, who admitted with pilonidal sinus disease to ***************, Department of General Surgery, underwent surgical therapy by lay open technique. The patients were randomized into two groups. the wound was cleaned daily with saline and povidone iodine and closed with gauze in group 1, and wound dressing with Hydrofiber dressing with silver were used 3 times a week in group 2. The demographics, number of wound dressings, complete wound healing time and wound care cost were compared.
RESULTS: There was no statistically signifi cant difference between the groups regarding to demographıcs. While wound-healing time was 61.15±21.4 days in control group, it was 41.05±10.9 days in group 2 (p<0.05). Mean number of wound dressing was 32.5±1.3 in group 1 and was 20.2±2.1 in group 2 (p<0.05). Mean wound care cost was 291.67±13.45 Turkish Liras (TL) in group 1 and was 260.35±56.12 TL in group 2 (p=0.32). Average follow-up time was 15 months. Only one patient had recurrence in the first group.
DISCUSSION AND CONCLUSION: Hydrofiber dressing with silver use in lay open technique of pilonidal sinus patients did not affect the cost of wound care. On the other hand, as an advantage, the wound healing time was statistically significantly shorter in this group. We believe it can be an alternative to conventional wound care especially in cases with large defects..

Pilonidal sinüs cerrahisi sonrası sekonder yara iyileşmesinde gümüş içerikli yara bakım örtüsü ile geleneksel tedavinin etkinliğinin karşılaştırılması

GİRİŞ ve AMAÇ: Burada eksizyon ve marsupiyalizasyon sonrası sekonder iyileşmeye bırakılan hastalarda geleneksel yara bakım yöntemleri ile gümüş içerikli yara bakım ürünlerinin etkinliğini karşılaştırmayı amaçladık.
YÖNTEM ve GEREÇLER: *********** Genel Cerrahi Kliniğinde PS tanısıyla opere edilen toplam 60 hasta çalışmaya alındı. Pilonidal sinüs tedavisinde tüm hastalara eksizyon ve marsupiyalizasyon uygulanarak yara sekonder iyileşmeye bırakıldı.. Grup 1'de (n=30) yara günlük povidon iyot ve serum fizyolojik ile grup 2'de (n=30) yara haftada 3 kez steril gümüş içerikli yara bakım ürünü ile pansuman edildi. Gruplarda demografik veriler, pansuman sayısı, iyileşme süreleri ve maliyet karşılaştırıldı.
BULGULAR: İyileşme süreleri grup 1'de 61.15±2.4 gün, grup 2'de 41.05±1.9 gündü (p˂0.05). Ortalama pansuman sayısı grup 1'de 32.5±12.3 iken grup 2'de 20.2±2.1 idi (p˂0.05). Grup 1'de maliyet 291.67±13.45 TL iken, grup 12de 260.35±56.12 TL olarak bulundu. Bu fark istatistiksel olarak anlamlı bulunmadı (p=0.32). Ortalama takip süresi 15 aydı. Sadece grup 1'de bir hastada nüks gelişti.
TARTIŞMA ve SONUÇ: Pilonidal sinüs ameliyatında eksizyon ve marsupiyalizasyon sonrası sekonder iyileşmeye bırakılan olgularda gümüş içerikli yara bakım örtüsü kullanımı yara bakım maliyetini artırmaz. Bununla beraber yara iyileşme süresini kısaltır. Özellikle defektin geniş olduğu olgularda klasik yara ba¬kımına bir alternatif olabilir.
