The Predictive Value of İntravesical Prostatic Protrusion on Prostate Histopathology

INTRODUCTION: One of the important markers of bladder outlet obstruction in benign prostatic hyperplasia is intravesical prostatic protrusion (IPP). In this study, we aimed to show the correlation between IPP and benign pathologies in patients who underwent transrectal ultrasound (TRUSG) biopsy due to high prostate specific antigen (PSA).
METHODS: Ninety patients who underwent ransrectal ultrasound guided (TRUSG) biopsy between January 2018 and January 2020 were included in the study. Age, PSA, IPP measure and pathology results of patients were recorded. The patients were divided into 3 groups according to the degree of protrusion. PSA values, pathology results, prostate volumes were compared statistically.
RESULTS: 30% (n: 27) Of patients had stage 1, 40% (n: 36) had stage 2 and 30% (n: 27) had stage 3 IPP. The mean age of the patients was 64.8 ± 7.54 (42-77) years and the mean total PSA value was 7.88 ± 4.75 (2-26) ng / mL. The incidence of prostate cancer among the groups was statistically significant (p <0.05). As the IPP stage increased, the incidence of prostate cancer decreased.
DISCUSSION AND CONCLUSION: The relationship between IPP and bladder outlet obstruction has been shown in the literature. However, the number of studies showing the relationship between prostate cancer and IPP is limited. In our study, it was seen that the average PSA value increased as the IPP degree increased, but the difference was not statistically significant. As the level of IPP increased, the incidence of prostate cancer decreased significantly.

İntravezikal Prostatik Protrüzyonun Prostat Histopatolojisini Öngörme Değeri

GİRİŞ ve AMAÇ: Benign prostat hiperplazisinde mesane çıkış obstrüksiyonunun önemli belirteçlerinden biri de intravezikal prostatik protrüzyondur (İPP). Biz bu çalışmamızda prostat spesifik antijen (PSA) yüksekliği nedeniyle transrektal ultrason (TRUSG) biyopsi yapılan hastalarda İPP ile benign patolojiler arasındaki korelasyonu göstermeyi amaçladık.
YÖNTEM ve GEREÇLER: Ocak 2018 Ocak 2020 arasında kliniğimizde PSA yüksekliği nedeniyle TRUSG eşliğinde biyopsi yapılan 90 hasta çalışmaya dahil edildi. Transabdominal USG ile İPP ölçümü yapılan hastaların yaş, PSA, IPP ölçüsü ve derecesi, patoloji sonucu kaydedildi. Hastalar protrüzyon derecesine göre 3 gruba ayrıldı. Üç grubun PSA değerleri, patoloji sonuçları, prostat volümleri ve bu verilerin İPP ile olan ilişkisi istatistiksel olarak karşılaştırıldı.
BULGULAR: Hastaların %30’unda (n: 27) evre 1, %40’ında (n: 36) evre 2 ve %30’unda (n: 27) evre 3 İPP mevcuttu. Hastaların yaş ortalaması 64.8 ± 7.54 (42-77) yıl ve ortalama total PSA değeri 7.88 ± 4.75 (2-26) ng/mL olarak bulundu. Gruplar arasındaki prostat kanseri görülme oranları istatistiksel olarak anlamlıydı (p<0.05). İPP evresi arttıkça Prostat kanseri görülme oranı azaldı.
TARTIŞMA ve SONUÇ: Literatürde İPP’nin mesane çıkış obstrüksiyonu ile olan ilişkisi çokça gösterilmiştir. Ancak prostat kanseriyle İPP ilişkisini gösteren çalışma sayısı sınırlıdır. Prostat volümü ve PSA seviyesi arasındaki pozitif korelasyon literatürde daha önce bir çok kez gösterilmiştir. Bizim çalışmamızda ise İPP derecesi arttıkça ortalama PSA değerinin yükseldiği görüldü ancak aradaki fark istatistiksel olarak anlamlı değildi. Gruplar karşılaştırıldığında İPP seviyesi arttıkça prostat kanseri görülme düzeyi anlamlı derecede azaldı.
